Registered Nurse Case Manager (RNCM)
Registered Nurse Case Managers (RNCMs) manage care plans, and ensure the highest level of support and well-being for individuals facing complex medical conditions. Here are key responsibilities and functions of RNCMs:
Conduct thorough assessments of patients’ physical, emotional, and psychosocial needs.
Collaborate with the interdisciplinary team to develop and implement individualized care plans.
Provide direct patient care, including administering medications, monitoring vital signs, and addressing any medical concerns.
Serve as a resource and advocate for patients and families through facilitating open communication and addressing questions and concerns.
Provide education and guidance to patients and families regarding their medical conditions, treatment options, and self-care techniques.
Monitor and evaluate patients’ progress, making necessary adjustments to the care plan to ensure optimal outcomes.
Facilitate smooth transitions of care, whether it involves home care, assisted living, or other appropriate settings.
