Registered Nurse Case Manager (RNCM)

Registered Nurse Case Managers (RNCMs) manage care plans, and ensure the highest level of support and well-being for individuals facing complex medical conditions. Here are key responsibilities and functions of RNCMs:

  • Conduct thorough assessments of patients’ physical, emotional, and psychosocial needs.

  • Collaborate with the interdisciplinary team to develop and implement  individualized care plans.

  • Provide direct patient care, including administering medications, monitoring vital signs, and addressing any medical concerns.

  • Serve as a resource and advocate for patients and families through facilitating open communication and addressing questions and concerns.

  • Provide education and guidance to patients and families regarding their medical conditions, treatment options, and self-care techniques.

  • Monitor and evaluate patients’ progress, making necessary adjustments to the care plan to ensure optimal outcomes.

  • Facilitate smooth transitions of care, whether it involves home care, assisted living, or other appropriate settings.
